ABSTRACT

ABSTRACT Purpose To develop an assessment protocol for speech motor planning with phonologically balanced stimuli for Brazilian Portuguese, including all necessary variables for this diagnosis. Methods Three stages were carried out: In the first, word lists were built with the main criterion being syllabic and accentual patterns. From the survey conducted in Stage 1, the words that composed the first version of the protocol lists in Stage 2 were selected, and grouped into two fundamental tasks for diagnosing acquired apraxia of speech (AOS): repetition and Reading Aloud (RA). In Stage 3, the occurrence of words was investigated using the Brazilian Corpus (PUC-SP) - Linguateca database, and a statistical analysis was performed to verify if the repetition and RA lists were balanced in terms of the occurrences. Thus, the lists were distributed in quartiles and submitted to both descriptive and bivariate analyses. A significance level of 5% (p<0.05) was adopted. Results After completion of all stages, the words that composed the lists of the repetition and RA tasks were obtained. Finally, other tasks considered essential for the assessment of AOS, such as diadochokinetic rates and the board for spontaneous oral emission, were then added to the protocol. Conclusion The developed protocol contains the tasks considered standard for the assessment of AOS according to the international literature, which makes this instrument important for diagnosing this disorder in speakers of Brazilian Portuguese.

ABSTRACT

ABSTRACT Autism Spectrum Disorder (ASD) is classified by Diagnostic and Statistical Manual of Mental Disorders (DSM-5) as a neurodevelopmental disorder, whose characteristics are mainly deficits in social communication and a restricted range of interests. There are several studies about autism, speech, and language in the literature, but few correlate speech and autism. This study aims to carry out a case study that will address autism, speech, and PROMPT (Restructuring Oral Muscular Phonetic Targets) and also to describe the speech improvement in the participant with autism using the method. The target words were defined for the entire intervention according to the System Analysis Observation (SAO) and Motor Speech Hierarchy (MSH), which are parts of the PROMPT evaluation. After the evaluation, the participant was attended for 16 sessions, once weekly, with the objective of improving their speech. After analyzing the data, it was possible to observe improvement in all aspects outlined according to the pre-treatment evaluation of the method such as phonatory control, mandibular control, lip-facial control and lingual control as well as in the sequenced movement although this was not the aim outlined in the evaluation. It was also possible to measure the improvement of an adequate number of words, an adequate number of phonemes, percentages of correct consonants - revised (PCC-R), and intelligibility.

ABSTRACT

Objective@#Analyzing the characteristics of consonant errors in children with functional dysarthria in different age groups and the effect of speech training provides a reference for clinical treatment. @*Methods @#This study followed medical ethics, and informed consent has been obtained from patients. Speech data from 388 patients with functional dysarthria were retrospectively studied. They were divided into two groups at the age of 6, namely, the preschool group (4-6 years old) of 226 patients and the school age group (6-13 years old, including 6 years old) of 162 patients. The characteristics of consonant pronunciation errors from four aspects were analyzed: average number of errors, pronunciation location, pronunciation method, and error type. One-on-one speech training was conducted, with a training frequency of once a week and once for 30 minutes. The training method was carried out in the order of phoneme training, syllable training, vocabulary training, sentence training, and short text and conversation training. The effects of speech training in the two groups were compared. @*Results@#Analysis by pronunciation location: both age groups had the highest frequency of errors in tongue tip posterior sounds; the school age group had the lowest error frequency for labiodental consonants, and the preschool group had the lowest error frequency for bilabial consonants. According to the analysis of pronunciation mode, both age groups had the highest error frequency of aspirated affricate and the lowest error frequency of nasal sound. Analysis by error type: both age groups are mainly characterized by substitution and omission. Compared with the preschool group, most consonants of patients in the school group tend to improve in terms of pronunciation location, pronunciation mode, and error types. Compared with the preschool group, the two types of errors-palatalization and lateralization-increased in frequency in the school group, but the trend of increased lateralization was not statistically significant. After 6.7 and 5.5 sessions of speech training, the pronunciation of the preschool group and the school-age group significantly improved; the cure rate of the school-age group was 84.9% (118/139), and that of the preschool group was 77.1% (91/118). There was no statistically significant difference in the cure rate between the two groups.@*Conclusion@# Functional dysarthria may improve with age, but it may not completely self-heal. Children of different age groups can achieve good treatment results through scientific and reasonable speech training.

ABSTRACT

ABSTRACT Purpose: to explore whether the diadochokinetic index of syllable production variability (DDKcvp%) is useful for the detection of articulatory inaccuracies in different stages of Parkinson's disease (PD). Methods: a diadochokinetic speech task of the consonant-vowel type [pa-pa-pa] was applied to 18 people with PD (classified in stages of progression from 1 to 5 according to the Hoehn & Yahr scale) to determine their diadochokinetic performance. Records of the study group were compared to a reference value obtained from a control group made up of 40 subjects. Results: 77.78% of the study group showed articulatory inaccuracies. The DDKcvp% index allowed to detect articulatory difficulties from stage 1 of PD, and its variation showed that articulatory deficits were permanent from stage 2 onwards. A significant and direct correlation was obtained between the increase in the DDKcvp% index and the increase in the stage of evolution of the participants. Conclusion: DDKcvp% index evaluated by means of a simple and fast consonant-vowel diadochokinetic task [pa-pa-pa], could be useful to determine articulatory inaccuracies in different stages of PD, although it is suggested that investigation of this index in larger sample sizes be conducted.

ABSTRACT

Speech is the vocal utterance of language and it is considered disordered in three underlying ways: voice, articulation, and fluency, its disorder is to be studied from same pattern. The features that should be considered in determining a voice disorder are: Volume: how loudly or softly we speak, Pitch: how pleasant or unpleasant Quality: the highness or lowness of one’s voice, Rate: the speed at which one speaks. Homeoapthic repertory shows these symptoms, but caution is mechanical problem should not consider as dynamic derangement.

ABSTRACT

ABSTRACT Joint stiffness is the most common complication in elbow injuries, presenting several etiologies and pathophysiological mechanisms that hinder treatment and prognosis. Prevention and treatment of joint contracture depend on the cause of stiffness, and early intervention should modify its outcomes. The methods used may be conservative or surgical, alone or in combination, according to each individual situation. Objective: Review articles on articulated elbow orthosis for joint stiffness rehabilitation. Methods: A literature review was conducted in journals available at the PubMed, Medline and LILACS databases, using the following Health Science Descriptors (DeCS): orthotic devices; braces; elbow; elbow joint; contracture; joint disorders. It sought to retrieve and analyze studies with the highest level of evidence that have already been conducted on articulated elbow orthosis for joint stiffness rehabilitation. Results: After applying the inclusion and exclusion criteria, four articles were included from PubMed and none from Medline or LILACS. Of the four PubMed articles, two were systematic reviews and two were randomized clinical trials. Conclusion: Articulated elbow orthoses can benefit joint stiffness treatment, improving range of motion and pain, showing superior effect compared to non-articulated plaster orthotics. Level of Evidence III, Systematic Review of Level III Studies.

ABSTRACT

Aim: to report two clinical cases of alteration of the lingual frenulum in twins, aged 10 years, both female, evaluated in the anatomical and functional aspects of the tongue before and 15 days after the frenectomy. Case reports: Initially, the paediatric dentistry performed the anamnesis with the mother. Then, the participants were submitted to a dental clinical evaluation and, sub-sequently, the speech language pathologist evaluation of the lingual frenulum was performed. The speech language pathologist assessment was performed using the Tongue Frenulum Evaluation Protocol, which was applied before and 15 days after the frenectomy. Both participants performed better in anatomical tests after the surgical procedure. However, some changes, especially when elevating the tongue, continued to be observed in one of the patients 15 days after the frenectomy. In addition, the two participants started to perform most of the tongue movements with greater precision. About speech, only the participant who had a greater number of previous phonetic changes, remained with some speech disorders after 15 days of surgery. Discussion: Some di-fficulties involving mobility of the tongue and changes in speech may persist after surgery, indicating the need for specific speech therapy intervention. Conclusion:Both cases showed improvement in performance both in anatomical and functional tests after frenectomy

ABSTRACT

ABSTRACT Purpose: To analyze the recognizability and effectiveness of items in the Phonological Assessment Tool (Instrumento de Avaliação Fonológica - INFONO) at eliciting target words, as well as to evaluate the internal consistency of the instrument and compare performance between genders, school types and typical vs. atypical phonological development. Method: Participants were 48 children (n=26 with typical phonological development and n=22 with atypical development) categorized by age (6 age groups ranging from 3 years and 8 years 11 months), type of school (public vs. private) and gender (male vs. female). Data were collected by the spontaneous naming task of the INFONO. Recognition rates, scores, recognition difficulties and internal consistency were examined in 116 items. Performance in a final set of 84 items was also compared between genders, school types and typical/atypical phonological development. Results: Most target words achieved high recognition rates were considered suitable for use in the INFONO. Some images had to be redesigned to facilitate the spontaneous production of target words, while other items were excluded from the instrument altogether. The instrument demonstrated excellent internal consistency. There were no statistically significant differences between genders and school types, though differences were observed between typically and atypically developing children. Conclusion: The images in the INFONO were successfully recognized by participants and were effective at eliciting the target words. The final set of items contained the minimum number of target words which would allow for an assessment of all phonemes in Brazilian Portuguese in different word and syllable positions, and these items presented excellent internal consistency.

ABSTRACT

(AU)

Temporomandibular dysfunction (TMD) can affect the masticatory muscles, temporomandibular joint (TMJ) and adjacent structures. The most common symptoms are pain in the TMJ region and chewing muscles, but in more severe cases can affect other facial regions that directly affect the quality of life of patients. Both surgical and non-surgical approaches may be used depending on the etiology and severity of the disease and the goal of treatment is to alleviate symptoms and thereby improve patients' quality of life. The aim of the present article is to describe a case where discopexy was performed as a surgical alternative in a patient who had unsuccessful conservative treatments to relieve facial pain, discussing the characteristics of joint disorders and treatment modalities. The patient continued with functional physiotherapeutic treatment and evolved without pain complaints, reporting improvement in quality of life. The surgical approach should not be considered the first choice when there is facial pain. However, under conditions of persistent and chronic symptoms, alternatives such as discopexy and temporomandibular joint surgery may be considered for the benefit of the patient... (AU)

ABSTRACT

Objective@#To access the influence of voice disorders on children′s voice-related quality of life through the parental version of pediatric voice handicap index (pVHI).@*Methods@#From April 2017 to March 2018, a total of 192 children with voice disorders (dysphonic group) and 111 children without voice disorders (control group) were enrolled in this work. Parents of children in both groups were asked for fill out the questionnaire containing the parental version of pVHI and the data of non-normal distribution were analyzed by Mann-Whitney U test. Spearman test was used for correlation analysis.@*Results@#(1)Vocal cord nodule was the most common voice disorder in children, and boys were more susceptible to voice disorder than girls in this study (70.3%(135/192) vs 29.7%(57/192)). (2)The most common voice abuse or misuse habit was "Shouting loudly". (3)In dysphonic group, the scores of function, physiology, emotion and total were higher than those in control group (all P<0.05). (4)In dysphonic group, there was a weak correlation between the parents′ overall evaluation of the children′s voice status and the three dimensions of the parental version of pVHI (function: r=0.339, physiology: r=0.334, emotion: r=0.208, all P<0.001).@*Conclusions@#Voice disorders can cause a negative impact on children′s quality of life. Parental version of pVHI can be used to assess the voice-related quality of life in children with voice disorders.

ABSTRACT

Objective@#To investigate the value of acoustic parameters in the voice therapy for patients with unilateral vocal cord paralysis (UVCP).@*Methods@#From May 2015 to April 2018, 51 patients with UVCP and 59 healthy controls in Department of Otorhinolaryngology Head and Neck Surgery, Tianjin First Central Hospital, were involved in this research retrospectively. The UVCP patients were diagnosed with stroboscopic laryngoscopy. The minimum glottal area (MGA) was calculated by KIPS software when the people were pronouncing/i:/. The fundamental frequency (F0), Jitter, Shimmer and NHR were detected by CSL4500 multiple acoustic voice analyzer.@*Results@#MGA of UVCP patients was much higher than that of healthy control (male: 433.68±64.52 vs. 294.41±51.82, t=9.23, P=0.000; female: 498.80±73.42 vs. 302.03±76.54, t=13.21, P=0.000), which meaned vocal cord insufficiency. After voice therapy, MGA reduced significantly (male: 288.48±55.09, female: 258.22±57.17, t=24.41 and 31.22, P=0.000 vs. pre-therapy). MGA of untreated patients decreased in varying degrees. Compared with the voice therapy group, the MGA decreased in a significantly lower extent (24.25±22.91 vs. 188.31±54.37, t=8.97, P=0.000). The F0, Jitter, Shimmer and NHR raised significantly in UVCP patients group (P=0.000 vs. healthy control group), and they were reduced by voice therapy (all P<0.05). Each of the four acoustic parameters was relative with MGA, r=0.551, 0.867, 0.853 and 0.875 in turn, P=0.001, 0.000, 0.000, and 0.000.@*Conclusion@#MGA and acoustic parameters can reflect the acoustic features of UVCP patients, which are useful tools in the UVCP assessment and voice therapy.

ABSTRACT

Objective To analyze the acoustic features of patients with Parkinson's disease (PD),and to explore the correlation between the acoustic features and the severity and course of disease. Methods Fifty-two patients with PD from the Tongji Hospital Affiliated to Tongji Medical University and the Ninth People's Hospital Affiliated to the Medical College of Shanghai Jiaotong University from August to December 2015 were enrolled into this study.Thirty-two age-matched healthy people served as control group.PD patients were tested with Unified Parkinson's Disease Rating Scale (UPDRS) and Hoehn-Yahr (H-Y) staging. And all the patients were tested with Mini-Mental State Examiantion (MMSE) and Voice Handicap Index (VHI).The voice samples of all subjects were collected and the frequency perturbation (jitter),amplitude perturbation (shimmer),and harmonic-to-noise ratio (NHR) were analyzed using a vocal assessment.Fundamental frequency (F0),standard deviation of fundamental frequency (SDF0) and formant ratio (F2i/F2u) were analyzed using a real-time speech measuring instrument. The acoustic parameters of the two groups were compared and the correlation between the parameters of the patients and the disease and course of disease was analyzed. Results Subjective assessment using VHI showed a total of 24 VHI abnormalities (46%) in the PD group, including 13 males (54%),11 females (46%).No VHI abnormalities were found in the control group.Among the parameters analyzed by objective acoustic method, compared with the control group (female/a/1.43(1.19,1.92),/i/3.39(1.49,9.85),/u/1.46(1.23,3.85);male/a/1.06(0.92,1.89),/u/1.30(1.07,1.64)),the SDF0 of the three vowels of the female patients in the PD group (/a/2.99(1.81,4.12),Z=5.429,P<0.01;/i/10.89(5.47,22.20),Z=8.487,P<0.01;/u/5.16(3.75,7.80),Z=7.138,P<0.01) and /a/ and /u/ of the male patients in the PD group (/a/2.16(1.73,2.94),Z=4.858,P=0.002;/u/3.70(2.41,5.43),Z=7.664,P<0.01) were significantly increased, and the F2i/F2u in the PD group (male 1.96±0.84,female 1.81± 1.14) was lower than that in the control group (male 3.48±0.70,female 4.14± 1.08),and the difference was statistically significant (t=-6.669,-6.844,P<0.01).There were no statistically significant differences in frequency perturbation, amplitude perturbation, harmonic noise ratio and fundamental frequency.Only the fundamental frequency standard deviation of the sound parameters used in the study was correlated with the disease course of PD patients. After the severity of the disease was assessed by H-Y staging, the frequency perturbation (jitter),amplitude perturbation (shimmer) in the middle and late stage patients were higher than those in the early stage patients, and the difference of frequency perturbation in the three vowels was statistically significant, and the difference of amplitude perturbation in the vowels/i/and/u/was statistically significant. No correlation was found between the acoustic parameters and UPDRS score. Conclusions Consonance disorders are common in PD patients, and the changes in acoustic parameters are mainly manifested as the increase in the SDF0 and the decrease in the F2i/F2u.Acoustic parameters can be used as an effective indicator to evaluate the condition and course of PD patients, and further language tasks need to be added to clarify.

ABSTRACT

ABSTRACT Objective: description and analysis of the phonological processes in the acquisition of complex onset by children with typical phonological development in the age ranges of 3;0 and 5;11. Methods: thirty-one students of a public day-care center in Maceió-AL, having no auditory, cognitive, or motor impairment, participated in this study. The data were collected using the ABFW Child Language Test (WERTZNER, 2004); spontaneous speech was also collected. Inferential statistical analysis of the data was performed, and the phonological processes were analyzed in the production of complex onset. Results: we found evidence for late acquisition of this syllabic component in the age range of 5;0-5;11 for both types of complex onsets. In relation to the prevalence of phonological processes, a statistically significant difference was observed between the different processes, with a prevalence of the simplified phonological process for C1V, when taking the two types of complex onset into consideration; however, the 5-year age group, in which there was a prevalence of liquid substitution, was an exception. Conclusion: the most observed phonological processes in children's speech were: simplification of C1V and liquid substitution. These results will contribute to the selection of the lexicon for evaluation and treatment of cases of atypical development.

ABSTRACT

ABSTRACT Purpose: to analyze the compensatory strategies used by subjects with ankyloglossia for the production of the consonantal alveolar flap sound /ɾ/. Methods: a cross-sectional analytic observational comparative study was conducted with 88 subjects, 44 being diagnosed with ankyloglossia and 44 with normal lingual frenulum, matched by age and gender. They were asked to repeat the syllable /ra/ five times in a row. Video recordings of the speech were taken. Frame by frame analysis of all recordings were performed to verify tongue, lip, and mandible movements during the production of the alveolar flap. The analysis of the films was performed by two Speech Language Pathologists specialized in Orofacial Myofunctional Disorder. Fisher's Exact Test was used for statistical treatment (p≤0.05.) Results: the frame by frame analysis of the alveolar flap production of subjects with and without ankyloglossia showed that subjects with ankyloglossia performed several compensatory strategies when producing this sound. There was a statistically significant difference (p=0.001) when subjects with and without ankyloglossia were compared. Conclusion: subjects with ankyloglossia used several lip, tongue, and mandible compensatory strategies to produce the Brazilian Portuguese consonantal alveolar flap /ɾ/.

ABSTRACT

ABSTRACT This study described the motor speech bases assessment and therapeutic process conducted through the hierarchy of motor speech treatment in a 45-year-old person, male, that has flaccid dysarthria caused by stroke. This patient received speech-language therapy after three years since the brain lesion, during 25 weekly sessions that occurred in 8 months. Speech-language assessments were applied before and after therapy, as well as a specific evaluation after each base motor treatment. Therapy obeyed the hierarchy of motor speech treatment, initiating with respiratory and resonance rehabilitation, following by prosodic therapy, phonatory treatment and, lately, articulatory treatment. The patient showed improvements in all motor speech bases, acquiring adequacy in respiratory support and resonance during the speech, improvements in prosody, more articulatory precision, and vocal stability. Beyond that, about patient self-perception about therapeutic progressions, he related reduction of the dysarthria impacts in his life quality. In this way, the benefits of speech-language therapy in dysarthria had been evidenced, mainly at following the proposal of the hierarchy of motor speech treatment structure. Results allowed us to conclude that an appropriate therapeutic approach may offer benefits even years after cerebral lesion.
